“Today, a partnership of voluntary sector organisations launched an online Inclusion Health Self-Assessment Tool. This simple and quick online tool is aimed at supporting Primary Care Networks (PCNs) and will highlight those people and communities within their populations whose voice they may not be hearing.
The tailored report received on completion of the tool, outlines many different resources and contacts to support PCNs to plan their approaches to reaching out to those communities and addressing health inequalities.

The tool was funded by NHS England and Improvement and developed by Friends, Families and Travellers, Homeless Link, Doctors of the World, National Ugly Mugs and Stonewall Housing, with input from Primary Care Networks, organisations specialising in work with Inclusion Health groups and people experiencing health inequalities.”
