Humber and North Yorkshire Inclusion Health Community of Practice

We would like to invite you to join the Humber and North Yorkshire Inclusion Health Community of Practice. This space is dedicated to connecting, collaborating, and sharing learning, best practice, plans, and activities related to inclusion health.
What is Inclusion Health?
Inclusion heath focuses on groups that are socially excluded and are at the extreme end of health inequalities. These groups include:
- People who experience homelessness
- People with drug and alcohol dependence
- Vulnerable migrants and refugees
- Gypsy, Roma and Traveller communities,
- People in contact with the justice system
- Victims of modern slavery
- Sex workers
- Other marginalised groups
These groups often encounter significant barriers to accessing and navigating the healthcare system, and we need to take action to develop and improve services to meet their needs.
Humber and North Yorkshire Inclusion Health Community of Practice
Our aim is to connect teams and foster collaboration across the system. We offer members a platform to share successes, overcome challenges and drive meaningful change.
Who should be involved?
We would like to cultivate a diverse and well-balanced community of members across Humber and North Yorkshire. The invitation is open to stakeholders from:
- Primary Care Networks
- Hospitals and Trusts
- Health and social care providers
- Local Authorities (i.e. Public Health, Housing, etc)
- VCSEs
- Other stakeholders who share an interest in driving action and innovation on inclusion health.
Upcoming meetings
Wednesday 15th October, from 3:00-4:30pm.
The first session will be hosted online by the Humber and North Yorkshire ICB and will be focused on Vulnerable Migrants.
