The following survey is asking for views and experiences of people living with dementia and carers/loved ones who are supporting someone living with dementia in North-East Lincolnshire.
The aim is to understand what works well and areas where there could be improvement. The survey responses will contribute to the lived experience information that is being collated from a focus group.
This work is being undertaken collaboratively by Alzheimer’s Society and Friendship at Home and the findings will be shared with commissioners to influence the requirements of a dementia support pathway.
