Centre4 are looking for a full time Social Prescribing Link Workers to connect people with local community activities, and services that can help improve their health and wellbeing, empowering people by focusing on what matters most, to them.
Participants will be supported by their Social Prescribing Link Worker though our 12-month program. The Link Worker will develop and manage their own caseload to ensure that reviews are carried out on time and regular contact with their participants. The role will be responsible for creating an action plan with participants to achieve their desired outcomes, and support them through delivery of the action plan, with staged assessments and redirection where appropriate.
